HAPPY NEW YEAR!!  After a two week hiatus....it is good to be back.  2012 will be an exciting year.  Lots of changes on the horizon for all of us.  Total Body Surgical will be entering its second year of service to Northeastern Oklahoma and the surrounding areas. Our success is all thanks to you.  Unless the Mayans are correct, we expect this year to be a banner one with many new changes to serve you better and MAYBE a few surprises!!

New changes are happening this year for us.  This installment is to explain them.  Next week we will get back into the meat.

First of all, we will be adding to our team in the next few weeks.  We will be adding an ultrasound tech.  Why??  Well, as many of you know, I have been doing the pre and post treatment vein ultrasounds and quite frankly, it has become too busy for me to do them and keep up with everything else.  Our new tech will specialize in venous studies and as things in the vein center continue to grow, will take over more responsibilities with insurance and the daily running of the center.  Venous disease is way underdiagnosed and the Northeastern Oklahoma, Arkansas and Missouri are underserved when it comes to the newer techniques in varicose vein treatments.  We offer them all and as the word gets out and we continue to get great results we will need more help.

Second, we will be adding a new person at the front desk.  This is greatly needed to serve you quicker when you come in, answer phones better, and allow us to provide the service you have come to expect at Total Body Surgical.  We strive to be the best at everything we do.  We hate to make people wait and know your time is important.  I feel that the addition of our new staff will help us get closer to our goal.  Be assured that these new faces will be just as friendly and nice as Mekala, Myranda, and Janet are now.... or we won't hire them!!

Third, physical changes.  As many of you know, our corner in Grove has been undergoing massive change for the better.  The old hospital is gone, the old intersection....gone...and the new shopping center is well under construction.  The intersection, which has hindered access for several months is now finished with new curbing and asphalt.  It looks great and will serve all of us much better than the old one....thank you Grove!!  We plan to make some cosmetic improvements to the outer office, and, as you know, the interior is very nice.

Fourth, I will be making more YouTube clips.  We plan to film procedures, pre and post op instructions and more.  We will attach them to the web site so that you will have a better understanding of what goes on during your procedure and help address your questions and fears.

These are the changes we are working on for 2012.  Keep and eye out for the surprises!!

The next few weeks I will talk a little about common General Surgical problems.  These may not be as fun as the Cosmetic and Vein discussions but are just as important, if not more so.

First we will talk about one of the more common problems...the gallbladder, then hernias, then endoscopy.  By that time we will be heading into spring.  I am working on some great pricing for "mommy makeovers" for spring that will include breast work, tummy tuck, and liposuction....very exciting.

Talk next week....Happy New Year!!

In the past few weeks we have discussed how lasers work, removing tattoos, removing unwanted hair, and renewing the skin.  These are important uses for laser technology and as it gets better, we will see more uses pop up.  Today I want to discuss pigment issues on the face, arms, and chest.

Why do we get them and what are they??melasma  is picture shows melasma, aka "pregnancy mask".  It is caused by sun in combination with hormonal changes.  It is quite common and cases can vary a great deal in severity.  These changes can occur on anyone but seem to be worse in ethnic skin.  It can occur over the entire face but is the most common on the cheek area.

chest pigmentationThe second area is on the chest.  Brown spots and red spots occur with age and again.....the sun.  Sun worshipers get this and the more you worship the worse it gets.

age spotsThe third area is on the hands.  Age spots, liver spots, whatever you want to call them are caused by sun damage and can be present over the entire arm.

Even if you don't worship the sun, we all get exposure during normal daily activites of walking out doors, driving, etc.  So they can occur on any of us.

How do we treat them??  First, I suggest a good skin care regimen.  This is key to getting the skin to heal and to slow down the pigmentation process.  It also prepares the skin for more aggressive treatments.  At Total Body Surgical we like to use the Obagi system for treating damaged skin.  Obagi contains tretinoin and hydroquinone.  These stimulate the skin to grow faster and shed the dead portion of the skin.  It also turns off the melanocyte or pigment cell.  Lastly, it contains a good sun screen to prevent further damage.  Once used for a short period of time, we treat these areas in a couple of ways.  First is the IPL (flashlamp or Intense Pulsed Light).  This is a machine that shoots short bursts of intense light.  The wavelength is set to target the darker colors in the skin and some red pigments.  These areas then peel off and since the pigment cells are turned off, their recurrence is much less severe.  Continued protection from the sun makes them "stay away".  Treatments are usually done in a series of 3-5 treatments and the areas gradually disappear.  We can also use laser to resurface the skin and remove pigment.  The Pearl laser is perfect for this killing "two birds with one stone".  This gives you rejuvenated skin texture, helps with fine lines and wrinkles, and rids the pigment too.  Recovery time for this treatment is only about 4-5 days.  The IPL recovery time is minor redness for about 24 hours then crusting of dark areas that will peel off.  The Obagi system causes the sensation of dryness and may cause some minor shedding of dead skin for a few weeks.

Come see us at Total Body Surgical where our staff is trained and experienced with Obagi and Skinceutical products.  We also have a full product line in the office and can get you started.

Remember our specials that are still going on too!!

What can lasers do for your skin??  ALOT!!!!  As time goes on and the natural aging process of the skin occurs there are many changes that occur.  Ultraviolet light from the sun, pollutants in the air, pollutants in what we eat, the loss of collagen as we age, genetics, loss of fat,....and the list goes on and on.  All of these things lead to the aging of our skin causing spots, wrinkles, enlarged pores, and even skin cancers.

uv light

Here is a comparison of young and old skin...here you see how the age spots form, the outer layer thins, and fat is diminished.  On a smaller scale we would see collagen loss as well.

skin    What we see on the outside is this....

aging  How can laser treatment alter these changes??  There are a number of effects that a good laser treatment can cause making the skin look and feel younger.  Laser can tighten the skin by causing new collagen to form as the damage caused by the laser is healed.  The outer layer of skin (stratum corneum) is also thinned and made more organized by laser treatment making the skin look and feel smoother and softer.  Dark spots (age spots) or dark patches (melasma) are not only caused by sun damage but also are aggravated by hormonal changes.  When lasers are used carefully and these treatments are combined with targeted skin care and sun protection, these spots can usually be treated and removed.

Laser skin resurfacing today is safer, cheaper, and recovery times are significantly less than ever before.  At Total Body Surgical we use the Cutera Pearl laser.  This laser is unique for a number of reasons.  It is a wavelength that penetrates well into the skin but shortens recovery time.  We have the only pearl in the area.  It gives the same results as a CO2 laser but the recovery time is only around 4 days.  This means that you can wear makeup after about 4 days but will have some redness for a week or two.

Laser resurfacing can make a huge difference in acne scarring too.  Smoothing them and helping pigment changes they can cause.

So if you want to get rid of age spots, melasma, acne scarring, or just tighten the skin up to look younger, come see us at Total Body Surgical.  We can get you started on the new you in 2012.  Now laser treatments are as cheap as $800....much cheaper than the big city.....schedule today!!   918-786-7780...cosmetic consultations always complimentary...

Don't forget our specials too!!

I want to spend some time discussing lasers and how we use them today.  Lasers have been around for some time.  In the past, at least as far as medicine is concerned, they have been a technology looking for an application.  In the past several years, improvements in technology have made them much safer and applications have greatly increased.

laser  Austin Powers....demonstrated his ignorance of laser technology.  Lasers have been a mystery to most!!

Laser and intense light or flash lamp therapy are both based on concentrated light.  Let's talk about lasers first.

laser technology

Lasers are concentrated light that can be used for multiple applications.  They have been used in industrial applications for years. Medical lasers have numerous applications as well.  We use them to to clear up and make skin look prettier.  We use them to remove tattoo's.  They are used to treat veins and vascular problems on the body and face, even varicose veins.  We can remove unwanted hair, laser fat cells, make skin contract, use them to cut skin, and now we can treat toenail fungus which has been a nagging problem.

Pulsed light therapy is based on a bright flash of light shot onto the skin.  Also called a flash lamp, this technology has been used for other applications, photography, etc.  In cosmetic surgery we use it to treat spots and different pigments on the skin of the face or body.

Unfortunately, there is no single laser that can be used for all applications.  Not to get too complicated (because it is complicated) the way we choose what laser or lamp to used is based on the chromophore we are trying to treat.  A chromophore is the part of a molecule or cell that produces its color.  Each laser or flash lamp has a wave length that attacks that color.  This is why one laser or lamp will not work on all colors.  We know what colors we are trying to treat and select the laser or lamp that will work on that specific color.  This is where the art meets the science.  With some applications, we are aiming at water in the skin as the chromophore, especially when resurfacing the skin.

The most common laser currently used in Cosmetic Surgery are the nd-YAG, the KTP, the CO2, and the pulsed dye lasers.  Each of these target a different chromophore or color.  The newest developments in the technology is the fractional and q-switched technologies.  We used to resurface skin by ablating or lasering the entire skin surface.  Since the skin recovers by healing from the hair follicle, recovery was slower than desired and this scared people.  Fractional lasers give us the ability to divide or fractionate the pulse of light from the laser.  This leaves islands of undamaged skin between the areas treated.  There are more and larger areas for the skin to heal.  The advantages are that healing is much faster and complication rates are less.  The disadvantages are that the treatment is not quite as good and may require more than a single session.  Q switching is a way to modulate the laser output to get a better "quality" of light energy, more intense output per pulse.  I will talk more about Q-switching when I discuss tattoo removal.

Flash lamps also treat specific colors in the skin.  They are used for treatment of brown pigment, red pigment, and hair in the treatment areas.  With flash lamps, we must change the wavelength of the light to treat each problem.

Now that the boring part is over, next week we will begin to explore the problems we treat and what we use to treat them.

What can I combine with Cosmetics? Total Body Cosmetic Surgery

I here it all the time....if I had only known, I would have had a tummy tuck when I had.....fill in the blank.  I must say that one of the great things about having experience and credentials in two fields (General and Cosmetic Surgery) allows me to offer a number of options that many cannot.  Of course Cosmetic procedures can be combined.  We do it all the time...tummy tuck with liposuction....face lift with eyelid surgery....tummy tuck with breast lift or augmentation....and the list goes on.  I will usually combine procedures as long as it takes less than around 4 hours.  I feel that this is a safe limit for anesthesia and for me this is about the limit of concentration for me and the surgery crew.  If I think adding procedures can be done in this time limit and it makes sense, we can do it.  If not, we divide the procedures into different days.
But what about combining cosmetic and Non-cosmetic procedures??  At Total Body Surgical we do them all the time.  It saves time and money for our patients.  One anesthetic and one recovery period gets people back to normal quickly instead of doing both twice.

The most common combination is a hysterectomy and tummy tuck.  This is an ideal combination.  The incision for a hysterectomy is low and transverse in the abdomen.  The tummy tuck incision is in the same area.  Hysterectomies are usually a fairly quick procedure and when combined with a tummy tuck the 4 hour time limit is more than enough time to do both, with considerable time to spare.  At Total Body Surgical we enlist (or are enlisted by) the OB/GYN surgeons Dr Thomas or Dr Dietz.  Most women having hysterectomies have had one or more children with the common pooch, stretch marks, and loose skin that goes along with it.  It is the perfect combination....one anesthesia....one recovery.

The second most common is hernias and a tummy tuck.  Many people have belly button, inguinal, or incisional hernias that need to be fixed.  These repairs can be done in conjunction with a tummy tuck very easily.  Depending on the size and location of the hernia I can either fix it laparoscopic or open.  The incisions are not made through the skin as the skin is moved out of the way during the tummy tuck.  The repair can be done with mesh if needed and AGAIN, one anesthetic and one recovery period!!

The third procedure is the cholecystectomy.  The most common general surgical procedure can easily be done in conjunction with a tummy tuck.  The skin is moved as with a normal tummy tuck, then the gall bladder is removed through laparoscopes and the tummy tuck is completed.  There is no sign of the procedure afterwards and only one anesthetic and one recovery.

Procedures that do not fit with cosmetic surgery are bowel surgeries.  These procedures are "dirty" procedures by nature due to bowel contents and just don't fit due to infection risks.

Sooo, if you were thinking about a tummy tuck and then had the bomb dropped that you also need a cholecystectomy or hysterectomy or hernia....come see us at Total Body Surgical and we may be able to save you time and money.  Check our portfolio page and see if you can pick which tummy tucks were combined procedures....there are some!!!

Sorry about the late post this week!  Spent the weekend in Oxford, MS at the Ole Miss, Arkansas game.  Oxford is a great place to visit and I recommend it to all.  The game was fun.  One of my daughters went to Ole Miss and the other is currently a Hog!!!

This week we are going to discuss the unusual uses of fillers.  Last week I talked about the normal uses around the mouth and in the nasolabial folds.  I hope you all got to read it.

There are a number of uses for fillers that are a little out of the main stream.  The first is in the nose.  There are a number of people that present with complaints of imperfections in the shape of their nose and they are not ready for rhinoplasty, OR, they have had a less than perfect rhinoplasty and have persistant defects.  In these cases I will use Radiesse or Silikon 1000 to help re-shape the bridge of the nose or fill defects.  With Silikon 1000 the fix is permanent and therefore I like to correct in stages so as not to overcorrect.  Fillers are a great way to a non surgical rhinoplasty.

A second out of the mainstream use of fillers is for cheek augmentation and filling the nasojugal fold....the line we get below the eye from the corner of the eye onto the cheek.  This is done in those that do not want an implant or fat grafting.  The filler is essentially placed where an implant would sit over the cheek bone.  It is a great way to rejuvenate the mid face without surgery.  It is done in the office and usually requires several syringes of product.  I usually use Radiesse as it offers more volume per syringe for the same price.  I like to call this the liquid face lift.  It is not perfect but does do a good job.

The third use is in rejuvenating the hands.  As we age, we not only lose fat in the face, we also lose it in our hands.  Many times we work on our face and body then forget the hands. Hands age quickly due to sun exposure.  Fillers or fat can do a great deal to make the hands look much younger.

The fourth place is for acne scarring.  Laser resurfacing can do a great deal to smooth out scarring that occurs from acne.  When they become too deep for the laser....fillers can help.  I usually use Juvederm or Silikon 1000 in very small amounts to help level these areas.  In some cases subcision or release of the scar must be done first and this is done in the office with some local anesthetic.

The last place I will talk about for fillers is the lips.  Believe it or not....they get smaller too!!  Adding volume to the lips is a great way to rejuvenate.  Voluptuous lips are what all the girls want!!  I like to use Juvederm, and for those who want a permanent fix...Silikon 1000 in a gradual fill.  Both are great and can make you look outstanding....

Enough about fillers I think.

Last week
we discussed (and hopefully dispelled) many of the myths about the use of Botox.  The FDA  has approved the cosmetic use of Botox in the  Glabellar region or the area between the eyes and forehead.

foreheadwrinkles                    botox before

However, off label uses for the medicine have become common place.  I like to use it around the eyes to elevate the brow, treat crows feet, and to open the eye to show more of the iris or colored part of the eye.  Botox is VERY effective in these areas and due to the small size of the muscles, it requires only a very small amount.

crows feet  Notice the "crows feet" around the outer aspect of the eye and the hypertrophy or enlargement of the muscle below the eye.  Botox works extremely well to smooth these areas creating a more youthful look.

Another area that can be improved is the neck.  Many men and women develop banding of the muscles that lead to wrinkling of the skin.  Botox relaxes the muscles that cause the banding and the skin smooths very well.

A third area is around the mouth.lip wrinkles

Many people start to form lines around the mouth due to the pursing motion of the muscles around the lips and loss of volume.  Smoking, using a straw, pursing the lips all create the lines.  When used carefully, Botox can help smooth these areas.  Add some volume and the mouth appears much younger.  Be warned that it may create some temporary weakness and this should be expected.  The chin area can also be smoothed with botox.

Another off label use is for Masseter muscle enlargement.

masseter hypertrophy

This can occur due to genetics or teeth clenching.  Botox can weaken the muscle and allow it to shrink giving the face a less angular look.

Botox can also be used to treat hyperhydrosis or excessive sweating.  The best area to treat is the under arm but it is also effective on the palms and feet.  Many people have this problem that cannot be resolved with anti-perspirants.  A simple test done at the time of treatment shows where the sweating is most profuse and small amounts of botox are injected.  It is very effective and seems to have good longevity....6 months to a year.

These are just a few of the off label uses for Botox.  There are many others and more are tried all of the time.

Next week I will discuss volume replacement with dermal fillers and other things such as fat or implants.

Be sure to check our liposuction, nail fungus, and lap band offers.  Many are taking advantage of them, saving significant money, and getting GREAT results!!

Watch this video for a little laugh....

http://youtu.be/nB7wtLMsm5c

This little cartoon displays many of the myths about botulinum toxin and its use in making us look younger.  Botox, made by Allergan, and Dysport, made by Medicis, have been around a long time.  Other similar products are on the horizon and these newer products will be just as effective and may drive down the cost.

For this blog I will refer to all of them as Botox as it is the most notable.

Botox was originally marketed for muscle disorders that caused spasm, disfigurement, and pain.  It worked very safely and very well.  As time went on, many of us realized that wrinkles in the face were caused by common muscles of expression that, over time, cause the skin that overlay the muscles to crease.  The areas most commonly noticed are in the forehead, between the eyes, and the crows feet around the eyes.  They are caused by us raising our brow as if surprised: forehead wrinkles, looking concerned: the "elevens" between the eyes, and smiling or squinting in the sun: crows feet.  It was noticed that when these muscles are weakened with Botox, the wrinkles were markedly reduced.  Over time, FDA approval was requested and granted for Botox use in these areas for cosmetic reasons.

Treatment with Botox started off being only for the "wealthy" as the cost was high.  Also, there were many misconceptions about the complications that could occur and fear due to rumors of deaths from the proper use of the product.

Here are the facts:

1.  Botox and other similar products are very safe.  The amount injected by qualified injectors is so small that there is essentially no risk of any serious complication.  As with any other injection, there is the small risk of bruising and if the medication is injected into the wrong muscle group there could be some brow droop or eyelid droop that is self limiting.

2.  Botox is not an anesthetic.  It does not cause numbness where injected.  It does weaken the muscles but when done correctly, they can still be moved, just not with as much force.

3.  Botox does not work immediately.  In some of the smaller muscle groups it works quickly (within a few hours), in the larger muscle groups it may take as long as a week.

4.  Botox is not permanent.  In the typical patient, it lasts about 4 months.  Some shorter and some longer.  The average patient receives Botox every 4 to 6 months.

5.  Some wrinkles may not disappear with Botox.  Deep wrinkles that have been present for some time will be softened after injections but probably will not go away.

6.  Botox does not "fill in" wrinkles.  It simply slows down the underlying muscle strength and if the skin is able, it will smooth out by not being wrinkled by muscle contraction.

7.  Botox is now becoming very affordable.  Prices, due to increasing competition, have fallen.  Also, as new products come to market, the price of the product should fall, allowing us to charge less.  The average price of a treatment now is around $250-$300 where it used to be twice that much.

8.  Treatment with Botox usually takes less than 10 minutes.  I use the smallest needle I can get and it is essentially painless.

So, there you have it!  Hopefully these points will help dispense many of the myths that surround Botox use.  It is one of the most effective medications I have ever used and patient satisfaction exceeds any other medication when used on the right people. The In and Out of Liposuction (no pun intended!)

Posted by Jeff Swetnam, MD on Sun, Sep 25, 2011 @ 10:34 AM

dr swetnamlogo

I am writing on liposuction today to introduce a fall pricing reduction at Total Body Surgical!  We are pros at this procedure and NOW is the best time to do it.  Why??? Well, liposuction does take some time to completely heal and "stabilize".  Getting ready for next summer starts today!  I have extensive training and experience with all types of liposuction and not to toot my own horn....am pretty good at it!

Liposuction has been around for a long time.  The first procedures were deadly in fact.  It was rediscovered and refined later but still often required blood transfusion and hospitalization.  Due to the work of Dr. Klein (a dermatologist), new and better transformations in the procedure now make it safe and more effective for the right people.

Who are the right people you might ask.  Liposuction works best for those who are "in shape" but have problem areas.  A chubby tummy, hips or love handles, saddle bags or outer thighs, inner thighs, chest, arms, or neck.  Liposuction IS NOT a weight loss procedure.  For those with smaller areas that they just cannot get rid of, liposuction is a procedure you should consider.

Modern liposuction can usually be done in an office procedure room.  We use sedation and tumescent fluid so that you can come and go the same day.  Tumescent fluid is the key to limiting pain and blood loss.   The technique, developed by Dr. Klein, is based on good science and is very safe when done properly.  The fluid is made using lidocaine (a local anesthetic), epinephrine (a normal chemical in the body that causes blood vessels to constrict or get smaller), and saline (salt water in concentrations that occur in the blood).  After marking the areas to be treated and sedation, the areas are injected with this fluid.  This causes the blood vessels in the area to constrict and numbs the area thus markedly limiting blood loss.  The areas fat is then removed with a small hollow tube.  This is done very carefully to not leave any defects or dents.liposuctionThe channels of fat removed shrink with time and the area smooths and recontours.  Without the fat!  Vaser and Laser liposuction are technologies that are used to heat the skin so that it shrinks to fit the smaller size of the underlying fat pad.  In my experience, using these technologies alone, without revoming the fat, does not give satisfying results and recovery times are about the same....a few days, then soreness for a couple of weeks.  The fat that is injured during the procedure and not removed is absorbed by the body.

The complications that can occur are:

-lumpiness (usually occurs if weight is gained after liposuction)

-bruising (resolves with time)

-sagging skin (the one variable that cannot be controlled.  Usually not a problem if the areas are selected appropriately)

-hyperpigmentation (especially in ethnic skin)

-dents (occur by removing too much fat in an area and can usually be repaired with revision)

Deaths can occur and are discussed by the media at length.  These usually occur when inexperienced physicians give too much lidocaine.  We are very meticulous about the dosing and when done properly, lidocaine toxicity is very rare.  However, any cosmetic procedure can have REAL complications and should not be taken lightly!  That's why I only do liposuction in the office on healthy individuals. I see patients personally, evaluate them, discuss the procedure's risks and benefits, and if I feel it is not the procedure to give you the results YOU desire, I will tell you.

Please go to my portfolio page to see some of my results and click on the picture or button to see our exclusive pricing on liposuction.  ACT NOW as this is a limited offer!!  Call 918-786-7780 for a complimentary consultation or fill out the contact form and we will call you....I look forward to meeting you!!

Lift or augmentation-good question! Total Body Cosmetic Surgery

Do I need a lift?? Todays blog is in response to a question I was asked on Facebook.  This is a question I hear almost daily.  It's a question many women are scared to ask.  The four most common cosmetic breast procedures are:

1.  Augmentation.

2.  Breast reduction.

3.  Breast lift.

4.  Breast lift with Augmentation.

So how do you decide what you need??  First, reduction.  Breast reductions are done cosmetically for women who have breasts that are too large for their frame.  They are so big that they interfere with life in general.  Clothing doesn't fit, bras are difficult to find, they can be embarassing.  They become a medical issue when neck pain, back pain, rashes, strap marks, etc. start to occur.  This is when insurances start to kick in and may cover the procedure.  They usually require certain criteria to be met and a certain volume must be removed for them to pay.  A reduction is done to reduce volume and make the breast smaller.  A lift is done to reposition the nipple.

Lift vs. augmentation vs. lift and augmentation.

The choice of procedure is at times a judgement call and also depends on what the patient is looking for.  It all depends on the position of the nipple relative to the infra-mammary fold. (the fold below the breast where the breast stops and the chest wall begins) breast nipple position If the nipple is well below this fold then a lift will usually be necessary.  If this breast is augmented, the nipple will still hang and the "Snoopy Nose" breast occurs, nice implant with a drooping breast over it.  The nipple must be raised and centered either in the breast or placed where it will be in the center of the implant.  If the nipple is above the infra-mammary fold then usually only an augmentation is done.  When the nipple is at the fold is when good judgement and figuring out what the patient is after, managing expectations, and all the other unmeasureables come into play.  For a lift, you will have scars.  They usually heal very well and after a little time are fairly inconspicuous, but they are there.breast augmentation(nipple above the fold-augmentation)

If a lift alone is done, the upper pole of the breast has a slope to it and may lack fullness.  Many women don't like that look and for them, an implant gives the breast a fullness that recreates the youthfull breast.

Notice the fullness in the upper pole after a lift and augmentation.
