Now that the breast special is over (just so you know, our usual price for saline implants is still the rock bottom price of $3800!) we're going to wear out the liposuction for a while.
"Lunch time" liposuction.....what the heck is that? Most people see liposuction as a very violent and traumatic experience, right? Here is a brief history of liposuction compliments of liposuction.com:
Liposuction has become one of the most commonly performed cosmetic surgeries in the United States. In 1974, Dr. Giorgio Fischer, a gynecologist from Italy invented the original form of liposuction. French physicians Illouz and Fournier further developed liposuction around 1978. By 1980, liposuction was extremely popular in the United States but was confronting negative publicity due to patients experiencing excessive bleeding and undesirable rippling of the skin after surgery.
In 1985, Dr. Jeffrey A. Klein, a California Dermatologist, invented the tumescent technique for liposuction, revolutionizing liposuction surgery. His “Tumescent Technique” allowed patients to have liposuction performed totally by local anesthesia using much smaller cannulas. Patients could now have liposuction surgery without the fear of excessive bleeding and undesirable skin depressions.
Here you can see the tumescent fluid (numbing medicine, saline or salt water, and a vasoconstrictor) being injected into the fat layer. This provides anesthesia for the area and helps to minimize bleeding. Of course, there is a limit to how much lidocaine we can use based on the patients weight and this limits the number of areas we can do at any given time. Even though larger areas can be done, we do them all the time, smaller areas are perfect for this technique.
We also use some sedation when doing larger areas to limit the amount of discomfort while the fluid is being injected. We then allow some time to pass so the fluid has time to work. Once the local anesthetic takes hold, we start the procedure. For most cases, we use the Cool Touch laser to break up the fatty tissue and warm the skin.
Warming the skin promotes new collagen formation, causing the skin to contract to its new position and tightening it.
We then use hollow cannulas to remove the liquified and non liquified fatty tissue.
This is a picture of a selection of cannulas in different sizes or diameters. The fat is suctioned out of the body, into a cannister, and discarded.
Once the fat is removed, it takes a few weeks for the swelling to go down and a few months for the skin to contract to its new position.
How painful is it?? This is everyones fear of course. For small areas, the discomfort is minimal, usually only putting in the tumescent fluid causes a minor amount. For larger areas there is some pinching around the edges usually and this is why we use sedation. People walk in and walk out, usually going home to rest after. After treatment of small areas, most people can return to work the next day and for larger areas, patients return to work after a few days or a week at most. You will be sore (like you did a very vigorous workout) for a few weeks and of course, the more you move around, the shorter the sore period is.
What you do after consists of a few things....first, exercise. Opinions differ but I like for patients to start stretching and massage after a week or so and resume normal exercise after about 3 weeks. Personally, I like for patients to wear a "spanks" for a month or so after large area liposuction.
I think it really decreases discomfort and bruising. Most people complain of an itching feeling on the skin over the treated areas as the nerves recover. My feeling is that the compression helps this sensation as well.
That's all for this week! Make sure you look at our liposuction offer...it is really a good one. If you have saddle bags, fluffy inner thighs, or any other area you simply cannot get rid of, give us a call today to schedule your complimentary consultation. Our pricing is the best.

Hard to tell by the weather, but Spring IS here, not only by dates, but soon the warmer weather too. In keeping with our Spring $3500 breast augmentation offer, we will discuss breasts. There are a little over 3 weeks left to take advantage, so click on the button below to get details and fill out the contact form. We'll get back with you. Or call 918-786-7780 or 1-866-weight0 to schedule an appointment that will be complimentary.
I think we have about beat this subject to death but wanted to go into a little more detail about approaches to implant placement and some of our thoughts. This will help YOU decide. You deciding seems to be a recurring theme here, but you need to happy with your choice of implants and how they are placed. Understand that we are happy if you are happy!! The size you choose, the insertion approach you choose is all about you, not us. We are here to help guide you in making your decisions. Dr Miller and I have a great deal of experience with these choices and can help guide you. Trish, Janet, Rachel, and Wendy also have experience, some have had this type of surgery and they are here to help too. They are also WOMEN, this gives them a leg up, as politically incorrect as that statement is!!
A couple of things that have been glossed over you need to know about....
1. Sizing: When it comes to the size of implant you want or need, there are several factors involved. Your height and weight. Your build, thin, athletic, or larger build, your current breast size, and your desired size after augmentation. We always hear comments on implant size based on what the internet says or friends comments, or the size someone else received. You can usually throw all of these comments and suggestions out of the window. Why?? You are unique. The amount of breast tissue you are starting out with is not exactly the same as everyone else. The size of your chest is not the same as everyone else. You are taller or shorter, thinner or fluffier than everyone else.....one size DOES NOT fit all in this situation. We start off by talking to you to get an idea what you want. We then measure your current breast location, how big the current breast is, and the size of your chest. This helps us to figure out a range of implants that will be right for you. If you have 100cc of natural breast tissue and want to increase to a 400cc breast, we need to look at implants around 300cc. If your friend started out with 300cc of natural tissue and ended up with a 200cc implant, she may think your 300 is "large" when if you consider where you both started....it is about the same...right?.....right. We will allow you to try different sizes on, in your clothes, so you can get an idea what you will look like. We will also help you choose the style of implant you want.
There is a new higher profile not pictured here.
2. Placement approach: How do you want the implant placed?? Also an important question. The options for saline are, armpit (transaxillary), below the breast (inframammary), through the nipple (peri-areolar), or through the belly button (TUBA).
Arm pit incision-saline or silicone.
The incision is only 2-4cm, not this big!! Saline or silicone.
incision around the nipple. Saline or silicone.
Silicone cannot be placed through the belly button.
We all have our preferences but we are here to help you decide what YOUR preference is. Here are the ones I don't like. Peri-areolar approach is less ideal because it can cause sensation changes around the nipple and leaves a scar on the breast. I also am not too fond of the TUBA. It has a higher incidence of assymetry and can also cause unsightly scarring around the belly button. My first choice is trans-axillary (through the arm pit), Dr Miller's is under the breast. There are several reasons why I like the arm pit...one is that there is no scar on the breast. I choose a small crease in the arm pit that is there in everyone and use it to make my incision. This usually heals well within 6-12 months and you can barely find it....no scars on the breast and the scar that is there is concealed. My second choice is the infra-mammary incision. This incision is probably the easiest one to use as it is the least remote from where the implants are placed. It is also a commonly used incision later if implants are replaced or any other work on the capsule is necessary. The down side....an incision on the breast, even though they usually heal very well.
So don't forget to call 918-786-7780 or 1-866-weight0 to make your appointment. With only a little over 3 weeks left, the time is now. As long as you book before April 31, you can pick any date after and still get the special pricing. Call TODAY!!

Mommy Makeover.....what's that?? Well, the definition changes for different parts of the country, but here in Oklahoma it usually means a breast lift (with or without augmentation) and a tummy tuck (with or without liposuction).
As most women know, after having children, the ole body has a tendency to change a bit. The hormones of pregnancy cause skin to stretch and in many cases, it never returns to the pre pregnancy tightness. The connective tissue under the skin also stretches, leading to the end of that flat tummy. We all see the movie stars that have babies and shortly after...LOOK GREAT!! Unfortunately, most of us have to work for a living and cannot spend our days at the gym or in a spa pampering our skin and re-toning our musculature. Even if we could do that, in many cases, our genetics make it impossible to shake the effects...in many ways, it's the luck of the draw.
To get back on point, after babies and breast feeding, the breast, which enlarged during milk production, shrinks and in many cases sags. The nipple position that used to be perky now points in a more Southern position and can lose volume.
Here is a picture that shows the different degrees of ptosis or droop. A or "normal" is perky, high and tight. After children, all bets are off as to where you will end up. My experience is that if you start off with little or no breast tissue, the post pregnancy changes are less dramatic than if you start off with a large amount of breast tissue.
Other issues can also occur with shape and in some instances breast abnormalities that have been present since birth. As you can see in the pictures below, chest wall shape, breast size, breast development, and nipple shape can all play a roll.
With all of this in mind, we try to help you return to your pre-pregnancy state if possible. This may or may not include implants, but what it does do is re-position the nipple and lift the breast tissue that is hanging. Some of this tissue may get removed if it is well below the fold where the breast hangs over the chest wall. My personal experience is that most women desire implants to increase the fullness in the upper pole of the breast. The incisions can differ depending on how far the nipple needs to move but usually they are in the form of an anchor or "lolly pop". The nipple is not "removed", it is rotated into its new spot conserving the nervous innervation and blood supply. These incisions have a tendency to heal very well and after 6-12 months they turn into little white lines. Implants, saline or silicone, are placed through the same incisions and usually under the muscle so they stay in place over time.
Now for the tummy..... We see stretch marks, sagging skin, bad c. section scars, bulging, hernias...you name it. The way to remedy all of these things is with the tummy tuck.
A tummy tuck is done through an incision in the lower abdomen a few centimeters above the labia. The incision goes from hip bone to hip bone and also around the stalk of the belly button. The skin is raised off of the underlying connective tissue and muscle up to the ribs. Once this is done, the loose muscle and underlying connective tissue is tightened back up to make the abdomen flat. The skin is then pulled down and the extra is removed. A new opening for the belly button is created. The smooth, non stretch marked skin from above the belly button is pulled down and stretch marked tissue, scars, etc. are removed. The incisions are closed and within 6-12 months they become thin white lines. Some can get away with a mini tummy tuck which removes a small amount of sagging skin (below the belly button) only.
Liposuction can be added to either procedure to remove problem areas in the hips, under the arms, thighs and saddle bags.
The great thing is this.....at Total Body Surgical, we can do both procedures at the same time, saving time and money.....one anesthetic, one recovery time. How can we do this?? Because we have two fellowship trained Cosmetic Surgeons that work very well together. We can do both procedures in under three hours and usually as an out patient.

To continue our discussion about weight loss, I wanted to cover, a little better, the procedures we are offering. As you know, not every procedure is the right choice for you. If you come in for a consult and we don't offer the one that is appropriate, we will send you to get the right one. The roux en y bypass, we will NOT be doing. This procedure is a great one for those who need rapid weight loss due to urgent complications of being massively overweight. We also will not be doing the so called mini gastric bypass as this procedure is unproven and not recognized by the Bariatric community.
The two procedures we will be offering are the gastric sleeve and the lap band.
The gastric sleeve has been around for some time now with great success and very low maintenance.
The gastric sleeve decreases the amount of food that can be consumed when eating a meal. It also decreases the amount of the hormones produced by the stomach that causes hunger and the desire to eat. This is a proven procedure that is done through the laparoscope with an over night stay in most cases. The weight loss is dramatic with no adjustments.
The lap band is for those people that can afford to lose weight in a slower fashion, and desire a procedure that does not change the anatomy in a permanent way. The band is also done through laparoscopes and generally does not require a stay in the hospital. Weight loss is slower with this procedure and it does require adjustments.
This photo shows the difference in the two procedures. The gastric band and the sleeve compare well at 5 years as far as weight loss is concerned, however, the speed of the weight loss is much faster with the sleeve procedure.
As the INTEGRIS bariatric program matures, we may offer the more aggressive roux en y but for now we are not.
We are striving to become a center of distinction in bariatric surgery and will take a little time. Until then we will not be able to treat medicare or medicaid patients as this is their requirement.
Dr Miller and I both are also fellowship trained Cosmetic Surgeons so once your weight loss stabilizes, if you desire any of the post weight loss body contouring procedures, we can help there as well. The most common are tummy tucks, breast lift with or without augmentation, arm reductions, thigh lifts, and butt lifts. Facial procedures are also common with face lifts being the most common. We are both very excited to offer these services. It is rare to find qualified bariatric and Cosmetic surgeons, not to mention in a small town, and in the beautiful Grand lake community.
Our plan is to start bariatric seminars within the next month in Rogers, Fayetteville, Bentonville in Arkansas and Joplin in Missouri. We will also have seminars in Grove and in the Tulsa area. The seminars will be informative and are required for insurance coverage of bariatric procedures. We will also discuss cosmetic procedures briefly at the seminars to give you an idea of what is available. You will be able to schedule appointments to start the process at the time of the seminars.
We are really looking forward to meeting many of you and hope that if you have any friends or family that are interested in weight loss procedures you will send them. I will send a schedule, as soon as we get it figured out, by email.
Don't forget our other special pricing on laser hair removal ($50/area/treatment), breast augmentation, liposuction, and nail fungus treatment. Press on the bubbles to see them. Schedule today to see Dr Miller or myself...918-786-7780.

Legs are often overlooked in Cosmetic Surgery. However, think about how much of your "person" is legs??? We all want to work on our faces, abdomens, and breasts but often forget about one of the larger parts of our body. What can be done??
Here are the common problems:
1. Lost tone and shape of the butt.
2. Saddle bags or outer thigh enlargement.
3. Inner thighs becoming large and "rubbing" together.
4. Weight gain in the anterior or front of the thigh and knees.
5. Spider veins.
6. Varicose veins.
These are the most common complaints I hear from day to day. These complaints are due to hormonal changes and the process of getting older. Also, genetics can play a role in how we all gain weight.
First, the butt. What can you do when the shape of the area is lost? The most common problem is not that the shape is lost, it has become hidden. Most of us gain weight in the hip or love handle area (the muffin top) and in the outer thigh or saddle bag area. When these areas enlarge, the but area becomes smaller in comparison. So how can this be fixed?? What I usually recommend is liposuction. Liposuction of the outer thigh and hip area makes them smaller and the secondary result is a more shapely butt. We can also take the fat from these areas (which is usually high quality) and transplant it into the butt. This will increase the volume of the buttocks and after liposuction of the surrounding areas, you can have the shape you are looking for. In this photo, you can see the improved shape of the buttocks after liposuction. It looks bigger because the surrounding areas are smaller.
Second, the Saddle Bag. These are the areas that not only "shrink" the butt, but also create a problem when trying to get into jeans and dresses. I hear it all the time, "I cannot buy jeans that fit my waist and thighs at the same time"!! Liposuction is what I use to attack these areas. It can be an amazing transition when you can drop 2-3 pant sizes with one in office procedure. Other procedures that are advertised may or may not work for you...this one is guaranteed. You can see from this photo that indeed, you can drop a significant number of pant sizes and regain your shape.
Third is the inner thigh area. I hear it all the time...."I hate that my thighs rub together". We can actually lump three and four together, the inner thigh, anterior thigh, and knee areas. Again, what we use is liposuction. These areas are fairly easily done and the recovery time is short. All of the areas can be done together to re-shape the entire upper leg. Small incisions about the size of the end of a pen are used in the groin area and in the fold at the base of the buttocks. Within a few weeks the difference will become visible and you will need to find some new pants...but don't, because, it will continue to get smaller for 4-6 weeks.
The next complaints are of veins. Spider veins and varicose veins are ugly. They are also, sadly, very common. Caused by heredity, trauma, child birth, and life style, they come and stay. They trap many people in long pants making it embarassing to wear anything else. Spider veins are considered a cosmetic problem. They are the small, blue veins that are very close to the surface of the skin. Treating them requires professional evaluation to make sure no other bigger problems with the veins exist. Once other venous problems are ruled out, they are treated by injecting small amounts of a substance to obliterate the vein. It does cause some inflammation and this procedure, called sclerotherapy, is best done in the fall and winter....NOW! It may take several treatments to get rid of them all, and may require maintenance. We charge $300 per session, very competitive. Varicose veins are a different story. They require more extensive evaluation and treatment. Much less than in the past however. Getting rid of these veins, when symptomatic, is done as an office procedure and is usually covered by insurance.
Come see us NOW to get ready for next summer....remember your legs....they need attention too!

Get ready for summer??? What!! That's right, this summer for all intents and purposes is OVER, it is now the time to consider what changes YOU want, and do them before it is too late for the next go around. There are things that you can do in fall and winter to be ready for lake, swim suit, and beach wear.
The first thing to consider working on are things you have a hard time hiding. Spider veins or varicose veins are two of the big things. I will talk more about options for treatment in the coming weeks, but, to have a great result and be ready for shorts and skirts, it takes time. The areas have to heal and may require several treatments. During fall and winter, long pants conceal treatment in progress....now is the time to start. Call to schedule..918-786-7780.
The second thing to consider is your complexion. Now is the time to get started on a good skin care regimen to reverse summer sun damage and keep that skin glowing through the winter. We love the results we see with the Obagi line of skin care and for those that need an extra kick, we use the Pearl laser and IPL to get rid of dark spots, fine lines, and wrinkles. The Pearl laser is fantastic for fine lines, wrinkles, and improving skin tone, color, and texture. It also comes with one of the shortest recovery time of all ablative laser treatments. Treatments in the winter will get you ready for next year...call today 918-786-7780.
The third thing, the second concerning lasers, is hair removal. With the lowest prices in the 4 state area, now is the time to get that unwanted hair GONE. Start now and stop shaving those sensative swim suit areas for good. With our pricing, you'll make money not buying those expensive razors....call today 918-786-7780.
Fourth...breast augmentation. Even though recovery time is fairly short for an augmentation, it is nice to get that over in the less active winter time. We can get you the look you want at a very fair price with short booking times. Call today for your consultation and book your time early. Restore your confidence next summer in swim wear...918-786-7780.
Fifth...liposuction. Have that abdominal pooch, love handles, or are you unhappy with your thighs? For the right people, liposuction is life changing. Problem areas that won't go away with exercise and diet can be sculpted into the form you are looking for. We have laser technology that can tighten skin as well. It takes time for the skin to tighten after laser liposuction so now is the time to get going. Consults are complimentary so call today..918-786-7780.
Sixth...toe nails. Many people are embarassed by their feet....toe nails in particular. Many of us have nail fungus that makes the nails thick, yellow, brittle, and discolored. Medications are 50% at best for curing the problem but come with many risks your liver. Laser Genesis has been FDA approved to treat the problem and is over 80% effective with only two treatments. It does take time for the sick nail to grow out. Do it now and get your two treatments out of the way, new nails...next summer....sandals!!! Call today 918-786-7780.
Of course there are many other things that you can do and it is always better to recover in the winter, tummy tucks, eyelids, facelifts, and more. The other thing that you can do to re-invent yourself is the lap band. If you are overweight and diets continue to fail, the lap band may be for you. If you do it soon, you could be fifty or more pounds lighter by next summer...would that be amazing or what!!! Book today 918-786-7780.
So there you have it!! My advice is to start preparing for next summer NOW. Don't wait or you'll miss it. We want to help and we have the skills and people to make it happen.

Many questions exist about liposuction. Most people have an image of a very violent and bloody procedure with results that are less than optimal. Todays liposuction is much different. Due to techniques we use today, liposuction can be done in the office, with only sedation. I would like to address a number of questions that are commonly asked of me during a consult, and many points that I like to talk about with patients to help them understand what they are to expect.
First, will I get fat in other areas after liposuction? This is one of the more common questions I am asked. The theory is that we are all born with a finite number of fat cells. As we gain or lose weight, we do not gain or lose these cells, they just grow or shrink in size. Some data show that in the super obese, cell numbers may actually increase but we don't liposuction these people. During liposuction, we remove fat cells in the area we are treating. Therefore, if someone gains weight after liposuction, it will accumulate in other areas since we have removed the fat cells in the area they have gained in before. This is true, however, to make a significant difference, it has to be a significant amount of weight....20-30 lb. Most people that spend the money on this type of procedure are not going to let this happen.
Second, will I get lumpy in the area of liposuction? This can happen, especially after weight gain. If you think about the procedure, we are removing layers of fat...not all of the fat. Some remains. This is a good thing. Leaving a layer of fat below the skin allows the skin to remain smooth and supple. Oversuction can create dents and allow the skin to scar to connective tissue underneath....both bad things. With some live fat remaining, if you gain weight, these pockets of fat cells will increase in size and can form areas of lumpiness. Lumps in the immediate period after liposuction are a normal thing. Usually they are a little sore and represent areas where fat has been damaged and the body is breaking them down. I like for people to massage these areas to speed up the process. It can take several weeks to months for them to completely resolve.
Third, will my skin be saggy after the procedure. This is the thing that is hardest to predict. The things I look for are the ones that indicate skin quality before the procedure. For example: The stretch marks in this photo would indicate to me that a tummy tuck would be a better choice for her...why? Her skin will have less of a chance of contracting after the fat is removed because of the poor starting skin quality. It will be more likely than not that her skin will not contract. This does not rule out liposuction for her but she must understand that, in clothes, she will look better and more sculpted, but in a swimming suit, the skin will still look bad and possibly more saggy. If her skin were smooth, she would be a much better candidate.
Fourth, will the newer technologies help the final result? Yes and no is the answer. Laser and ultrasound technology have helped but they are not the silver bullet. I like to use them as an adjunct to traditional liposuction. The idea is that heating the skin from the inside does cause some contraction, how much is the unknown. I remove the fat first then use the laser to heat the skin to allow skin contraction. Several studies have been done that indicate the use of these technologies alone are sub-optimal and my experience verifies this.
We do liposuction often and combine it with many procedures. It is a great procedure and helps a lot of people. We do the procedure in the office with sedation and tumescent solution. This solution contains lidocaine that numbs the area and also epinephrine that causes the small blood vessels to contract and limits blood loss. Patients recover quickly and this depends on the size of the treatment area. It takes the skin several months to contract and heal so the final result is seen after 3-4 months.
Come see us for a consult and for a limited time receive 10% off of the procedure. Don't forget our laser and breast offers too!!!!

Today I am going to give you a test. Fun, huh!! No grades will be given, thank you. Hopefully you can learn something from this and if you like it, let me know here and I will come up with some similar ones for other common procedures.
Today the test is going to be about liposuction and tummy tucks....when is one appropriate and not the other and vice versa. Good luck!!
#1. ....what do you think?? Tummy tuck or lipo?
We start with an easy one. If you said tummy tuck, you would be right. There is no doubt there is a lot of fatty tissue present and liposuction COULD be considered, however, the hanging skin would only become worse. A tummy tuck with associated liposuction of the waist would be the appropriate choice. The tummy tuck would remove the excess skin and stretch marks below the belly button and liposuction of the waist would accent and renew the shape of the sides of the abdomen.
#2. Which is it, tummy tuck or liposuction?
This one is a little tougher. When you look at this abdomen what do you see? As far as fatty tissue goes, there is only a problem area in the mid abdomen and a lower abdominal pooch. Normally, this would be a perfect abdomen for liposuction, however, you always must evaluate the skin quality too. The skin in the lower abdomen is riddled with stretch marks. For this reason, I would recommend a tummy tuck. The tummy tuck removes a majority of the stretch marks, tightens the remaining ones and moves them below the pant and swim suit line. Of course if she presented not caring about skin quality after the procedure, a liposuction procedure could be selected. I would doubt the level of satisfaction after liposuction alone.
#3. Which one??
Not exactly a clinical photo, but, it makes a point. This girl is the perfect example for liposuction. Perfect skin, nice shape overall, and just a small area of fat in the lower and mid abdomen....what a pallet!! I could make this abdomen look amazing. I would use a small cannula, remove the fatty tissue and give her some definition. I would then use the cool touch laser to tighten the skin more. It would look fantastic.
#4. The last one...Tummy tuck or lipo....this one is a little tough so take your time!
This abdomen is a little tougher. The skin looks pretty good. The shape is fair with some fatty pockets that could easily be removed with liposuction. Liposuctioning the waist would also improve her shape into a more hourglass look. BUT, what about the C-section scar? It is a point of adherence that liposuction will not address. I would recommend a mini tummy tuck with liposuction. This would allow me to tighten the lower abdominal skin, remove the C-section scar, and liposuction the waist. I would not make an incision around the belly button but remove some skin that will make it look better.
Good job! I hope you enjoyed our little quiz. Remember to let me know on facebook, twitter @jeffswetnam, or here if you like it. I can come up with some more I think you will like.
Here are a couple of pictures of my work....
Don't forget our laser hair removal special...$50.00/treatment/area...amazing. Also, our breast augmentation special for silicone and saline. Lap band specials as well.
Welcome to Trish, our new RN. An amazing girl that we are very proud to add to our Total Body Surgical Staff.

I am writing on liposuction today to introduce a fall pricing reduction at Total Body Surgical! We are pros at this procedure and NOW is the best time to do it. Why??? Well, liposuction does take some time to completely heal and "stabilize". Getting ready for next summer starts today! I have extensive training and experience with all types of liposuction and not to toot my own horn....am pretty good at it!
Liposuction has been around for a long time. The first procedures were deadly in fact. It was rediscovered and refined later but still often required blood transfusion and hospitalization. Due to the work of Dr. Klein (a dermatologist), new and better transformations in the procedure now make it safe and more effective for the right people.
Who are the right people you might ask. Liposuction works best for those who are "in shape" but have problem areas. A chubby tummy, hips or love handles, saddle bags or outer thighs, inner thighs, chest, arms, or neck. Liposuction IS NOT a weight loss procedure. For those with smaller areas that they just cannot get rid of, liposuction is a procedure you should consider.
Modern liposuction can usually be done in an office procedure room. We use sedation and tumescent fluid so that you can come and go the same day. Tumescent fluid is the key to limiting pain and blood loss. The technique, developed by Dr. Klein, is based on good science and is very safe when done properly. The fluid is made using lidocaine (a local anesthetic), epinephrine (a normal chemical in the body that causes blood vessels to constrict or get smaller), and saline (salt water in concentrations that occur in the blood). After marking the areas to be treated and sedation, the areas are injected with this fluid. This causes the blood vessels in the area to constrict and numbs the area thus markedly limiting blood loss. The areas fat is then removed with a small hollow tube. This is done very carefully to not leave any defects or dents. The channels of fat removed shrink with time and the area smooths and recontours. Without the fat! Vaser and Laser liposuction are technologies that are used to heat the skin so that it shrinks to fit the smaller size of the underlying fat pad. In my experience, using these technologies alone, without revoming the fat, does not give satisfying results and recovery times are about the same....a few days, then soreness for a couple of weeks. The fat that is injured during the procedure and not removed is absorbed by the body.
The complications that can occur are:
-lumpiness (usually occurs if weight is gained after liposuction)
-bruising (resolves with time)
-sagging skin (the one variable that cannot be controlled. Usually not a problem if the areas are selected appropriately)
-hyperpigmentation (especially in ethnic skin)
-dents (occur by removing too much fat in an area and can usually be repaired with revision)
Deaths can occur and are discussed by the media at length. These usually occur when inexperienced physicians give too much lidocaine. We are very meticulous about the dosing and when done properly, lidocaine toxicity is very rare. However, any cosmetic procedure can have REAL complications and should not be taken lightly! That's why I only do liposuction in the office on healthy individuals. I see patients personally, evaluate them, discuss the procedure's risks and benefits, and if I feel it is not the procedure to give you the results YOU desire, I will tell you.
