When you come to Total Body Surgical for a breast consult, the first thing that will happen is a discussion about implants.  We cover about every positive AND negative to having implants and contrast and compare the saline with the silicone gel implants that are currently available.  We feel it is very important for you to have all of the knowledge.

First is saline.  Saline implants were made popular when the silicone implant scare started in the 90's.  These implants are a bag that is placed empty and filled with saline.  They can be placed through an incision under the breast, through the arm pit, through an incision around the nipple, or through the belly button. If a lift is required, then we use the same incisions.  We usually discourage using the nipple access for obvious reasons.  When placed throught the belly button, the incidence of assymetry is reportedly higher.  We generally use the arm pit or an incision under the breast.

The pros to the saline implant are several.  One thing that we can do with them is over fill them.  This makes them look like implants.  Some women want this.  Another advantage is if they rupture.  If this happens, the saline is absorbed and the implant goes flat.  We simply replace the implant and you go on your way.  Because saline implants are put in flat, they can be placed through a smaller incision whichever approach you decide upon.  Most studies support a lower capsular contracture rate with saline implants (this is when the implant is distorted by a thicker and tighter capsule that makes the implant feel hard and can cause distortion).

Lastly, and in many cases most importantly, saline implants are cheaper.  They are about $1200 cheaper than silicone implants.

Now the cons.  Some people think that saline implants don't feel as "natural" as silicone implants.  Secondly, saline implants are generally placed under the muscle to avoid some of the limitations of the implant.  Thirdly, saline implants have a higher incidence of "rippling".  Ripples are creases that can form in the implant.  They are more prevalent in women who are very thin and get larger implants.  This can occur with silicone implants as well but less so.

Now for pro's of silicone gel implants.  Some women think they feel more "natural" than saline.  Some women think they have a more "natural" shape.  I think, in the moderate profile shape, they both can look great.  In the high profile shapes, it is my opinion that the silicone gel is the implant of choice and looks better.  It is also my opinion that when implants are placed above the muscle, silicone is the implant of choice due to its lower pressure.  It ripples less and looks better.  Below is a video of what a silicone gel implant looks like when you cut it open.

The con's.  You must be 22 (the infamous FDA) to get them.  Silicone gel implants are full when we put them in, therefore, the incision must be slightly larger than with saline.  Studies show that the capsular contracture rate is slightly higher than with saline.  Silicone gel implants are more expensive than saline. Implants made of silicone cannot be placed through the belly button.

Skin Care and Hair Total Body Surgical Cosmetic Surgery

I started working in the yard last weekend.  It was warm and sunny.  While I worked, I noticed that the trees are budding out, and there were even a few with some juvenile leaves showing up...guess what...the warm days of summer are nearly here!!  While we talk about hair and skin, don't forget about our breast augmention special, hit the button below to get the details.

This week I wanted to cover a couple of old subjects...skin and hair.  With summer rapidly approaching, these are two of the things that you can improve quickly and get ready for summer.  So what can you do??

1.  Hair.  Most women (and some men) don't like hair in certain places.  This includes the bikini area, underarms, face, and legs in women, and chest, back, and underarms in men.  What can we do about this?  Have you ever shaved in any of these sensative areas and wished you didn't have to? We use the Cutera system to do our laser hair removal.  It is one of the few systems that is FDA approved to treat all skin types and it is very effective.  We expect to get rid of 80-90% of the hair completely and what is left is spindly, light, and weak if there is anything.  Of course the hair has to be darker for us to treat it because that is how the laser works...the dark hair transmits the light energy as heat into the follicle, killing the hair stem cells.

As the population of these cells decreases, the hair becomes thinner and thinner until eventually, it is gone. Body hair generally takes about 6 treatments and facial hair, because it is thinner and transmits less energy, about 9 treatments.  The darker and coarser the hair, the quicker it goes.  The treatments are spaced about 6 weeks apart and generally take about 30-40 minutes.  We use numbing cream and let it set about 20 to 30 minutes before the treatment.  If you do not want to numb it takes less time of course.  Everyone is different so some like the numbing and some don't.  We use the Zimmer cooling system so the discomfort is minimal even if you don't numb.

Remember, hair has to be there for the laser to work.  This means you must wait six weeks after waxing or using a cream hair remover.  You can trim or shave the areas being treated so it does not have to be long and embarrassing.

The way we do areas is like this....the face is considered an area, the underarms is considered an area, the bikini is considered an area.  The legs are considered 4 areas due to the large size so lower leg front and back is 2 areas, thigh front and back is 2 areas.  The abdomen upper and lower is 2 areas and the chest, upper and lower, is 2 areas.  The back, upper and lower is condered 4 areas as well, divided as the abdomen and chest are.

2.  Skin.  Most skin issues, i.e., pigment changes, are due to UV (sun or tanning bed) exposure.  In women, hormonal changes also contribute.  These changes have many names....

Of course, they occur on the sun exposed areas of the face, chest, neck, and arms.  If you have these areas, they will usually NOT resolve by doing nothing.  What do we advise??  First, sun protection.  No matter what we do, if sun exposure continues, these areas will return.  Sun screens, hats, and cover ups are critical to the success of any therapy and once you have that covered, we can start addressing the problem.  Basic skin care is critical and the product we recommend is Obagi.  Created by a Dermatologist the line is medical grade and is VERY imitated.  It contains medical concentrations of tretinoin and a lightner.  The kit we recommend also contains the products to manage the pH of the skin, making the medicines work better.  Compared to other products, it is very affordable.  After a few months you will see the skin looking healthier and the discolorations decreasing.  For faster results, or for areas that are a problem, we use Pulsed light treatments.  These treatments target areas of pigment and cause them to peel off quicker and also help turn off the cells that are creating the pigment.  It usually takes a series of treatments along with the basic skin care.  I feel like the best sun screens come from the Skinceutical line of products.  They were also created by Dermatologists and come in many strengths and even in a water proof formula.

HAPPY NEW YEAR!!  After a two week hiatus....it is good to be back.  2012 will be an exciting year.  Lots of changes on the horizon for all of us.  Total Body Surgical will be entering its second year of service to Northeastern Oklahoma and the surrounding areas. Our success is all thanks to you.  Unless the Mayans are correct, we expect this year to be a banner one with many new changes to serve you better and MAYBE a few surprises!! 

New changes are happening this year for us.  This installment is to explain them.  Next week we will get back into the meat.

First of all, we will be adding to our team in the next few weeks.  We will be adding an ultrasound tech.  Why??  Well, as many of you know, I have been doing the pre and post treatment vein ultrasounds and quite frankly, it has become too busy for me to do them and keep up with everything else.  Our new tech will specialize in venous studies and as things in the vein center continue to grow, will take over more responsibilities with insurance and the daily running of the center.  Venous disease is way underdiagnosed and the Northeastern Oklahoma, Arkansas and Missouri are underserved when it comes to the newer techniques in varicose vein treatments.  We offer them all and as the word gets out and we continue to get great results we will need more help.

Second, we will be adding a new person at the front desk.  This is greatly needed to serve you quicker when you come in, answer phones better, and allow us to provide the service you have come to expect at Total Body Surgical.  We strive to be the best at everything we do.  We hate to make people wait and know your time is important.  I feel that the addition of our new staff will help us get closer to our goal.  Be assured that these new faces will be just as friendly and nice as Mekala, Myranda, and Janet are now.... or we won't hire them!!

Third, physical changes.  As many of you know, our corner in Grove has been undergoing massive change for the better.  The old hospital is gone, the old intersection....gone...and the new shopping center is well under construction.  The intersection, which has hindered access for several months is now finished with new curbing and asphalt.  It looks great and will serve all of us much better than the old one....thank you Grove!!  We plan to make some cosmetic improvements to the outer office, and, as you know, the interior is very nice.

Fourth, I will be making more YouTube clips.  We plan to film procedures, pre and post op instructions and more.  We will attach them to the web site so that you will have a better understanding of what goes on during your procedure and help address your questions and fears.

These are the changes we are working on for 2012.  Keep and eye out for the surprises!! 

The next few weeks I will talk a little about common General Surgical problems.  These may not be as fun as the Cosmetic and Vein discussions but are just as important, if not more so.

First we will talk about one of the more common problems...the gallbladder, then hernias, then endoscopy.  By that time we will be heading into spring.  I am working on some great pricing for "mommy makeovers" for spring that will include breast work, tummy tuck, and liposuction....very exciting.

Final Comments on Breast Implants By Dr Jeff Swetnam

Very funny huh!! Problem is, most women stress about how to pick implants. There are many questions that must be answered before making a life altering decision. Questions that you should ask are:

1. Saline or silicone, what are the risks and benefits?

2. Approach, how should I have the implants placed?

3. Size and how they will look IN me?

4. Price, how much can I afford?

5. Recovery, how long will it take?

6. Implant warranties....what are they?

7. Should I place the implants above or below the muscle?

8. Do I need a Breast lift? 

At Total Body Surgical we have a check list that we cover with every patient to cover all of these questions and more. I will answer the questions in a short fashion here but you should ask your Doctor as his or her opinions may differ.

1. Saline or Silicone....I like them both. Some women prefer the feel of silicone as they think they look and feel "more Natural". Some women have a fear of silicone. I let them feel both, explain the cost difference and let them decide. I cover what I have discussed in previous blogs about the implants.

2. Approach.....we do them all. Silicone can be placed around the nipple, under the breast, through the arm pit. Saline can be placed through all of these approaches and through the belly button as well. I prefer the arm pit as I can use creases in the skin that heal well and are concealed leaving no scar on the breast. I let you decide however.

3. Size. As explained before, we let you help choose. I will give you a range based on the size of your chest and let you try them on. My bias is this...if a Doctor tells you he will decide based on experience what looks best for you....don't walk....run from his office. This is an important decision that you should be involved in. You try on clothes don't you????

4. Price. Saline implants are usually about $1000 cheaper than saline. This is because they are more expensive to make, and carry more liability for the manufacturer. In my office, it is up to you to choose. I explain the benefits of each and you try them on. Unless you want a high profile implant, you decide based on look and feel. High profile implants are a bit different as I feel that the silicone have a far superior look.

5. Recovery. Recovery is usually a few days. I like for you to be careful lifting or raising your arms for around 2 weeks but pain is minimal after the first 3 days if you have any at all. The implants will find their home after about 1-3 months if under the muscle and shorter if above.

6.  Implant warranties, what about them?  Both of the major manufacturers will cover the cost of the implant if there is a mechanical failure due to a manufacturers defect.  This does not pay for the surgery to replace them.  They both do have a very inexpensive insurance policy that will cover this cost for 10 years.  The implants are also covered for 10 years.

7. Above or below the muscle. I usually put them below the muscle. I put them below with saline, always to avoid seeing the implant or ripples in the implant. I put silicone above the muscle in women with a slight bit of droop of the nipple.

8. Do I need a lift?? The answer to that question is complicated. Usually if you look at the fold under the breast where it attaches to the chest wall and the nipple is above this fold, you probably won't need a lift, if below, you may. However, there are other factors involved that you will need to consult your physician about.

These are the most commonly asked questions.  Hope this helps.  Breast augmentation is a very personal event.  You should have a big say in how it is done.  One of the biggest problems I see is when a girl comes in with a centerfold picture and her body is very different.  This is an expectation that cannot be fulfilled.  BE REALISTIC!!

Breast Implants Option 1-Saline

Breast implants in society today are very common.  It used to be a procedure limited to the very wealthy but now is within reach of pretty much anyone who wants it.  Over the years there has been a great deal of change in the availability and quality of implant materials.  There are more, but the main manufacturers of implants today are Mentor and Allergan.  Both are great companies and both make good, solid, reliable implants.  Silicone implants I will cover in a later blog and will spend all of my time today talking about saline implants.

Saline implants basically are a silicone shell that is filled at the time of surgery with saline.  Saline is sterile water with sodium chloride added.  It is very prevalent in our bodies and very safe.  The implants are filled to the pre set volume that is chosen at the time of consultation.

Saline implants can be placed through the nipple, under the arm, below the breast (inframammary) or through the belly button (TUBA).  There are no age restrictions on saline implants and if they leak, they just go flat and your body absorbs the saline.

The down side to saline is that they may Ripple or wrinkle more and they tend to look more round.  Some think they feel hard.  I have seen GREAT looking saline implants and I don't push people away from them if cost is an issue or if they have any silicone fear.  Silicone implants are more costly than saline.  Incisions need to be slightly larger for silicone as they are pre filled and the saline implants come empty.
