Let's have some fun today. It's been a little while since we had a little audience participation and I think it is time to change that. Sooo, it's test time again!!
Here are the rules:
1. Look at the picture and see what you think would help. (those of you who come here often will find it easy...you are educated!!).
2. The choices will be between using Botox, a Filler of some sort, laser, or a surgical procedure.
You may elect to combine the use of these tools and that is OK. I do it all the time. Today though, let's not get too complicated. In my analysis of the patient I will give you some ideas on what other things would be good for the patient. I need to feel useful in some way!!
Always remember, there is no right answer, because, in Cosmetic surgery everything is elective. You can always choose to do nothing. However, there are wrong answers. Wrong answers are when you plan to do more than the patient needs or wants. My job is to steer people away from unecessary procedures and only do what's needed or desired...today, that's your job. Good luck!!
Example #1:
Here we have a 40'ish female with no other medical problems. She complains that she looks "old" and wants a face lift or something to help her look the way she feels. Look at her closely. What do you think??
What would you recommend:
1. Botox 2. some type of filler 3. laser 4. a surgical procedure
Example #2:
40'ish young lady who complains about the lower part of her face sagging. Her friends have told her that you have helped them and she should come talk to you about what could be done. You would recommend???
Your recommendation??
1. Botox 2. a filler of some type 3. laser 4. a surgical procedure
Example #3:
A 54 year old lady who is disgusted with her neck and just thinks she looks OLD!! She says that cost is no obstacle and wants it all fixed, or injected, or something!
And the winner is....

Example #4:
28 year old female who states her boyfriend says she has "wrinkles around her mouth". She is concearned about this and wants to know what you think....
Here she is...

Example 5:
A 65 year old widow comes to you and says she has finished mourning the loss of her husband and wants to look younger so she can "get out there".
Very nice lady....funny too....what would you recommend??

Well, let's see how you did. On number one if you said Botox, that would be a great start. Most people want to start small. Botox would get rid of the lines around the eyes and in the upper face. She will eventually probably want a brow lift to eliminate the lines and the cause of them but Botox is fairly cheap and less invasive.
On number two, I would say Botox and some type of filler. This combination is great for softening wrinkles in the upper face and getting rid of nasolabial folds as this lady has. She has good facial structure and her skin is in good shape. This will make her very happy and not cost a fortune.
On number three, I would recommend a surgical procedure or two. Looking at the "hooding" over her eyes, she would benefit greatly from a brow lift. It will take years off of her appearance. She also has the "turkey gobbler" neck and jowling but her neck is not heavy. I would recommend a mini lower face and neck lift as well. She will be very happy!
On number four....get a new boyfriend of course. We see people like this sometimes and my job is to DO NOTHING. She needs to be told how beautiful she is and just how silly it would be to do anything to change that. Unfortunately, in many cases, these people will find someone to mess things up. It's sad.
Number five would need a brow lift, upper eyelids, face lift, and I would also recommend full face laser to treat the discolorations and fine lines on the rest of the face.

Last week we discussed how Botox can be used to prevent some wrinkles and soften others. It is a great product and is now more affordable than ever. Today I would like to briefly discuss what to do about those wrinkles that won't go away with Botox or those that Botox is not appropriate for....what can we do with them??
The most common complaints I see about facial appearance AFTER Botox have to do with residual wrinkles or sunken spots around the mouth. Here is a common picture that shows and labels the areas we are discussing. As we age, the face loses volume..ie fat. We get it where we don't want it and lose it where we'd like to keep it....go figure!
When the volume is lost, of course the skin falls a little creating a hollow area. When not too severe, these areas can be helped tremendously by replacing the volume with either harvested fat or some other material.This picture demonstrates a before and after of a nasolabial fold filled with Radiesse....Nice result I think. The area is softened quite nicely. This is done in the office with very little discomfort and an immediate result. We could also use Juvederm in this area. Both fillers last a year or more and there are few problems with them.
Another area commonly complained about is the Marrionette line area.
Here we see in the before picture how the nasolabial folds are prominent and the marrionette lines are too. These lines are present at the corners of the mouth and cause the lip corners to turn down. When filled, the result is pretty amazing. You get a much more youthful appearance with no surgery and no recovery time.
A third area complained about is the glabellar area. Botox works to soften this area but when the lines persist, it is a great place to fill.
These three areas are the most commonly treated spots that we use fillers on. At Total Body Surgical I use Juvederm and Radiesse. I think they both work very well. As I said, they both work for about a year. We do injections in the office and it takes very little time. There is no recovery time and the worst problem that can arise is a bruise.

The past two weeks we have discussed the common and uncommon uses for Botox and similar neurotoxins in the management of facial and neck wrinkles.
The next question is "what are my options when Botox doesn't work"??
The next easy step to correcting a wrinkle that is too deep for Botox is a Dermal Filler. Traditionally, fillers are used for diminishing nasolabial folds as pictured here....
These are lines that form from the lower edge of the nose to the lateral edge of the mouth. They occur as fat is lost from the mid face and the skin begins to sag. Lipoatrophy, or fat loss occurs with age and hormonal changes in us all. This volume should be replaced, and that is what dermal fillers are used for.
The second most common area is the oral commissure or Marrionette line at the lower lateral edge of the mouth. In this picture there are many other areas shown and fillers can be used for most of them with glabellar lines, perioral lines, and lip augmentation rounding out the most common uses.
What are fillers anyway? Dermal fillers have been around for some time. They have gotten progressively better with time, the biggest improvement has been the length of time they last. Most modern fillers stay around for a year or more and some are made to stay around forever.
One of the most common fillers is Juvederm.
It is made of hyaluronic acid (a normally occurring substance in the body) that has been cross linked to make the molecule very large. This makes it hard for the body to digest it so it stays around longer...up to 12 months. It also has a numbing agent within the product to ease the discomfort during injection. Juvederm and other hyaluronic acid fillers can be used almost anywhere in the face including the lip.
Another common filler is Radiesse Radiesse is made of Calcium hydroxylapatite in a sugar base that promotes collagen development when injected. It cannot be used in certain areas of the face but is a great product for the more common areas except lips. It is mixed with lidocaine to make injections nearly painless as well.
I think both of these products are very good and at Total Body Surgical we use them both, sometimes in combination.
Of the permanent fillers, I like Silikon 1000. Silikon 1000 is made to use in the eye but due to its high quality, it can be used as a filler if used carefully. Of course it is off label and not FDA approved. I like to use it to volumize lips and to treat some acne scars. It works great when done properly and is permanent. This can be very economical over time.
Next week I will talk about the more unconventional uses for fillers....some you may not have thought of.

For the final installment on wrinkles or rhytids we are going to cover procedures for people that other easier methods will not work for. We have discussed skin care and sun avoidance for prevention, then Botox to rid ourselves of lines that are not yet permanent. For areas that Botox will not soften, we use fillers such as Radiesse and Juvederm. After these products have worn out their ability to improve we move into laser skin resurfacing. When all of these tools fail....then what? The answer is the facelift. Sometimes only a surgical correction will do. As we lose collagen and fat in our face and the connective tissue below the skin sags, the skin follows and becomes lose. There are some non-invasive methods that may help, but minimally. Face lifts come in many different types and one size does not fit all. The oldest types only address the lose skin and have little longevity, they fail in no time because the skin stretches quickly and wrinkles and loose skin return. Don't believe that skin stretches quickly??? Talk to a pregnant woman, skin can stretch to an incredible size, and can do it quickly. Real face lifts address the underlying connective tissue, tightening it, and then remove the excess skin. There are several types of lifts that are commonly used and the procedure should be individualized for each patient. The one picked should address the specific areas that need to be lifted. The mini face lift (s-lift, lifestyle lift, short scar lift, etc.) addresses the lower face, jowel area and some lose skin in the neck. The vector of pull on the connective tissue and skin is up and toward the ear. The incision starts at the temporal tuft of hair in front of the ear, follows the front contour of the ear, down around the ear lobe, and a short distance behind the ear. This lift does have a shorter recovery time and works great for the appropriate patient. Those with a heavy neck will need a "full lower face and neck lift". This lift has the same incision but is is extended onto the back of the ear and into the hair line. The vector of pull for the connective tissue and skin is not only up for the lower face, but is back and up for the neck as well. Usually there is also a small incision under the chin to remove fat and sometimes tighten or remove some of the underlying muscle tissue. During face lift surgery, the underlying connective tissue is tightened to correct the loosened structure that causes the face to droop and some of the fat is sculpted or liposuctioned too. Then the skin is draped and trimmed then carefully sutured into its new location. Facelift surgery should also address underlying bone and fat deficiencies too. This may require cheek or chin implants, fat grafting, or other techniques. A good face lift should not leave the patient looking stretched, it should not disrupt the normal smile or look fake. The incisions heal very well over time and are difficult to detect. You should look younger and well rested. When looking for a face lift surgeon, demand to look at pictures and be sure that areas that bother you will be addressed. Also, make sure the pictures you look at are from patients done by the surgeon you are talking to...this is an old trick...claim anothers work for your own.

Last week on the Total Body Surgical Cosmetic Surgery blog we talked about wrinkles and how good skin care and Botox can make a big difference. This week we will take a look at the common fillers that can be used.
Below is one of the commonly used scales to measure and quantify the severity of wrinkles and skin damage.

Class II we would usually use Botox or Dysport because slowing the underlying muscles will usually eliminate the wrinkle as long as it is maintained.
Class III wrinkles will usually require "filling" as they are present at rest and are more or less permanent fixtures. Filling them will not remove the wrinkle 100% but will go a long way toward making them tolerable.
There are many fillers on the market and thank goodness most of them now last a year or more. This is in contrast to only 5 or 6 years ago when a filler would work for 2-4 months. Many techniques can be nearly peramanant.
Hyaluronic acid fillers such as Juvederm and Perlane are great all around fillers that can be used nearly anywhere in the body. This is because it is a naturally occurring substance in the body and is colorless. They cause very little collagen production and the newer ones contain lidocaine which is a nice plus for the patient and injector. These are the most widely used products.
Radiesse is another filler that is made of small calcium hydroxyapetite beads in a sugar based carrier. This filler is limited in its use and should not be used in the lip or superficially anywhere. It is great for larger volume fills in the cheek and nasolabial fold. It works by causing your body to form collagen in the treated areas. It should last 12 to 18 months but will need some supplementation in that time.
Other common fillers are Sculptra which is poly L lactic acid can be used to create volume in the cheeks but this is a much slower process than the other two.
More permanent fillers are fat and silicone. Fat harvesting and grafting has become common all over the body, but is very popular in the face. Fat is harvested, treated and placed in areas where fat has disappeared over time. There is some variability in the percentage of the fat that lives and therefore the procedure may need repeating. Lastly, silicone. Silicone has received a bad name. There are countless examples of low grade silicone used in too large a quantity to create a terrible result. At Total Body Surgical, I have a great deal of experience with silicone. We use only the best quality silikon 1000 that is manufactured by Alcon and made for use in the eye. It works great for augmenting lips, filling areas of imperfection on the nose, and some acne scarring.
