I hope everyone had a great 4th....time to get back at it.
One of my friends asked me to address those unsightly veins that show up on the face.
Here is a good example of what I'm talking about. These are from a condition called rosacea that causes inflammation and eventually the larger veins that appear on the cheeks and around the nose. The problem is usually made worse with heat (i.e. working in the yard during summer) or consumption of red wine. It can occur in all ethnic groups but is much more visible in the lighter skinned groups. Rosacea is treated with topical medications, oral medications, and light therapy which will be covered later.
Rosacea can also have a deleterious effect on the color and shape of the nose. Known as "rhinophima", the nose becomes red with acne like lesions that eventually cause the nose to become misshapen. It can also cause very large veins to become visible.
The veins that appear at the base of the nose are very common as we age. Some people have them worse than others of course, but lighter skin tends to allow them to show up more prominently. They can occur with or without rosacea.
Here is another example of a facial vein that sometimes bothers people. This is known as a facial reticular vein. They can occur around the eye and temporal area. They are present in young and old alike.
In some instances, instead of visible veins, just a "redness" is seen. This probably is just early rosacea and should be treated as such.
How do we treat these problems? The treatment depends on what the cause is and how severe the problem has become. With rosacea, we treat it topically and sometimes with low dose antibiotics when it is early and large venous areas are not present yet. Once outbreaks are controlled, the redness usually subsides. If it does not, the next step would be to use Intense Pulsed Light treatments to close the small capillaries that are flushing and causing the skin to turn red. These treatments (also known as IPL) use bright flashes of light of a certain wave length to damage and close the small vessels that are causing the problem. Usually several treatments are required, sometimes up to 5. These treatments will controll the problem for some time after, but usually not permanently, so treatments may be required every few years or so.
For the larger veins, in the cheek, nose, and base of the nasal area, we use the vascular setting of the Cutera 1064 YAG laser. Using a very small spot size, the veins can be closed and they are no longer visible. In some cases, we will use both the laser, and the IPL in combination. This treats the redness that is generalized and the larger veins that are too large. The treatments are generally pretty quick and the discomfort is tolerable. We use numbing cream for most as well as the Zimmer chiller that blows cold air over the treated areas. For larger treatments we will call in Valium if needed.
Reticular veins around the temples or eyes are also treated with the YAG laser. They may be a little sore after treatment for a few weeks until the body resorbs the vein and some bruising is possible though not very common.
The nasal effects of rosacea can be severe if left untreated. The laser is used to control the venous problem and it helps with the sebaceous problem as well. Left alone, the nose can turn into a trunk so treatment is necessary.
All of these treatments are done in the office with little or no sedation. The treatments alone are fairly quick, taking only a few minutes. Usually there is only a mild amount of redness after, though there can be some areas of ulceration or skin slough. These areas usually heal very well. Ideally, you come in and have the treatment done, leave with a little redness, after that resolves, the veins and redness are gone or greatly diminished. As I said before, more than one treatment is usually required and are done 4-6 weeks apart to allow for healing.

Get ready for summer??? What!! Happy Father's day to all the dads out there and the dads of the future!

Laser days at Total Body Surgical!!!! We know have the Cutera laser full time in Grove...VERY exciting. The Cutera laser is one of the most comprehensive lasers, IPL combinations on the market. The system can nearly do it all, and do it well. I have many years of experience using this system and it is great at treating a large number of problems. Let's itemize them:
1. Laser hair removal. With the Cutera system we use the ProWave 770. It is an IPL (intense pulsed light) that can treat large areas and do it very effectively. I guess the test last week was not so good judging from the response. A few people really loved it so I may consider doing another in a few months....maybe on breasts...lift vs augmentation alone....or....faces, mini vs. full face lift....we'll see.
Today I want to discuss the Pearl laser. Lasers have come a long way in the past 10 years. We have gone from a technology looking for a procedure to actually being able to make a huge difference in a number of skin issues. The great thing is that recovery times and safety have jumped in leaps and bounds.
There are many reasons to choose laser skin resurfacing. What resurfacing can do is:
1. Reduce facial skin discolorations such as brown "age spots" and other discolorations. These are caused by sun damage and age.
2. Reduce fine lines and wrinkles in the face, neck, under the eyes, and crows feet.
3. Shrink pores that are enlarged.
4. Improve acne scarring.
5. Improve skin color and texture.
6. Improve vertical lip lines.
Why the Pearl?? The Pearl laser is fairly new technology that is similar to the CO2 laser. What makes it good? There are a number of reasons the pearl is superior. First is recovery time. The recovery time from the average treatment is only about 4 days. This means that new skin reforms quickly, much quicker than the CO2. Second is cost. The cost of the Pearl treatment is $1200. What is IPL? It is a great way to remove unwanted discoloration or dyschromias on the face, chest, hands...anywhere on the body. It is the use of Intense Pulsed Light or a flash lamp to influence the pigment cells in the skin.
How it works is by using a specific wave length of light that targets certain color in the skin. The color we usually are after is brown. Age spots, liver spots, whatever you want to call them, are a problem caused by skin aging and sun damage. They are present on sun exposed parts of the body. The IPL is absorbed by these brown areas that then peel off and are gone. The melanocyte (the pigment cells in the skin) is encouraged to turn off and when the skin is then protected from future sun exposure, they usually stay away.

We like to combine good quality skin care with Obagi products to speed up the effects of IPL and make them longer lasting. The Tretinoin and hydroquinone causes the skin to grow faster and the hydroquinone turns the melanocyte off. The IPL gets rid of the current brown spots and voila your skin begins to look like it did before the sun and baby oil did its damage!!!
Red spots on the skin can be caused from sun damage as well. Also causing the problem are small blood vessels in the skin. To treat these areas the wave length is different but the process is the same. The small blood vessels are injured by the light, closed, and the vessels are broken down by the healing process. The red areas are removed.
Usually, IPL treatments are done in a series of 3-5 treatments. Once problem areas are gone, protection from the sun is the key to long term success.
A treatment consists of about a 20-30 minute application of a numbing compound. We wash that off and the actual treatment takes only a few minutes. Hands can be treated as well and most want the decollatage or chest area treated too.
We use the Cutera XEO system which allows us to use multiple wave lengths or combine wave lengths in a single treatment. At Total Body Surgical we charge $150 per treatment and we have special pricing for packages of multiple treatments which is usually required.

Last week we talked about eyes, the window to the soul. Today we are going to discuss another part of the anatomy that can instantly tell your age.
Youthful lips are generally very full and inviting. They are made of the white line where the skin and the pink mucosa of the lip join and the fullness of the lip takes off. The cupids bow is in the center of the upper lip giving definition and contrast that the lower lip is missing. Usually the lower lip is slightly larger than the upper. Youthful lips are smooth and surrounded by skin that is also smooth with uniform color and texture.
So what happened?? A number of things. Skin aging, sun damage, volume loss, i.e. fat, around and in the lip, smoking, using straws, smiling, frowning, kissing and just time. All of these things age our entire face but since the mouth is one of the centers of motion and expression in the face, like the eyes, it shows the use as we age. The lips become thin and surrounded by vertical lines, nasolabial folds, marrionette lines. The "white line" becomes less visible and the "cupids bow" becomes a straight line.
How can we repair the damage?? We can lift the tissue around the lips, correcting the cheeks and neck. We can correct the nasolabial folds by adding volume in the form of a filler (Juvederm, Radiesse, fat grafting). We can do dental correction to improve the underlying structure if needed. To correct the skin damage, it is important to improve the skin. We can do this in a few steps. First, good skin care is critical. As stated here before, we recommend Obagi products that combine Tretinoin and hydroquinone to correct color changes and improve the texture of the skin. We then can augment the lips using temporary fillers like Juvederm or more permanent fillers like Silikon 1000. This replaces the volume that has been lost. Lastly, after pre-treating the skin with Obagi and repairing sun damage, the deeper wrinkles and vertical lines remain. What to do about them? They can be filled if extremely deep, again, with a temporary filler. The best way to treat the area is with laser. Resurfacing the area promotes new collagen production, softens the wrinkled areas, and removes the sallow color of the skin. I usually recommend doing the entire face along with the area around the mouth. This removes transition zones and looks more uniform. We use the Pearl laser which is great for this. It has a faster recovery time than similar lasers, can be spread over multiple treatments, and gives great results. The price for the first treatment is only $1000 and repeat treatments are $500. Most require 2 or 3 treatments for a great outcome. The order should be, skin care, laser, then filler as some laser treatments can degrade the filler material.

HAPPY NEW YEAR!! After a two week hiatus....it is good to be back. 2012 will be an exciting year. Lots of changes on the horizon for all of us. Total Body Surgical will be entering its second year of service to Northeastern Oklahoma and the surrounding areas. Our success is all thanks to you. Unless the Mayans are correct, we expect this year to be a banner one with many new changes to serve you better and MAYBE a few surprises!!
New changes are happening this year for us. This installment is to explain them. Next week we will get back into the meat.
First of all, we will be adding to our team in the next few weeks. We will be adding an ultrasound tech. Why?? Well, as many of you know, I have been doing the pre and post treatment vein ultrasounds and quite frankly, it has become too busy for me to do them and keep up with everything else. Our new tech will specialize in venous studies and as things in the vein center continue to grow, will take over more responsibilities with insurance and the daily running of the center. Venous disease is way underdiagnosed and the Northeastern Oklahoma, Arkansas and Missouri are underserved when it comes to the newer techniques in varicose vein treatments. We offer them all and as the word gets out and we continue to get great results we will need more help.
Second, we will be adding a new person at the front desk. This is greatly needed to serve you quicker when you come in, answer phones better, and allow us to provide the service you have come to expect at Total Body Surgical. We strive to be the best at everything we do. We hate to make people wait and know your time is important. I feel that the addition of our new staff will help us get closer to our goal. Be assured that these new faces will be just as friendly and nice as Mekala, Myranda, and Janet are now.... or we won't hire them!!
Third, physical changes. As many of you know, our corner in Grove has been undergoing massive change for the better. The old hospital is gone, the old intersection....gone...and the new shopping center is well under construction. The intersection, which has hindered access for several months is now finished with new curbing and asphalt. It looks great and will serve all of us much better than the old one....thank you Grove!! We plan to make some cosmetic improvements to the outer office, and, as you know, the interior is very nice.
Fourth, I will be making more YouTube clips. We plan to film procedures, pre and post op instructions and more. We will attach them to the web site so that you will have a better understanding of what goes on during your procedure and help address your questions and fears.
These are the changes we are working on for 2012. Keep and eye out for the surprises!!
The next few weeks I will talk a little about common General Surgical problems. These may not be as fun as the Cosmetic and Vein discussions but are just as important, if not more so.
First we will talk about one of the more common problems...the gallbladder, then hernias, then endoscopy. By that time we will be heading into spring. I am working on some great pricing for "mommy makeovers" for spring that will include breast work, tummy tuck, and liposuction....very exciting.

Today, for the final installment on lasers, we are going to discuss the use of lasers to treat veins on the face, legs and elsewhere.
First, leg veins. Superficial or spider veins occur very frequently on the legs. As I have discussed before, when the areas are large, I prefer injections to the laser because I can cover much more ground and cause less dicomfort. For small areas, however, the laser is very convenient and in most cases works very well. The veins in this picture are perfect....why?? Remember that laser light is attracted to pigment. The darker the pigment, when the laser light hits it, heat is generated, the small vein is injured, the flow stops and the vein is gone. After lasering the vein it turns red and forms a clot that slowly dissolves. I use the Cutera 1064 laser for the darker veins. For veins that have a lighter and more red pigment we use the KTP or 536 wavelength laser.
The second area that is most commonly complained about is the face. Adults of European descent commonly develope visible veins around the base of the nose that are unsightly. Those with Rosacea get veins that are visible on nose, at the base of the nose, and nearly anywhere else on the face.
The nasal areas we treat with the laser to close the veins. For Rosacea, it may require several different methods, even systemic medications on some cases.
The results we get are very good. In most cases, new areas will arise and will require repeat treatments from time to time we call maintenance.
A third condition that is vascular and can be treated with laser are hemangiomas. These areas occur in the young and old and anywhere on the body. They can be very small. They can also be very large. Most of these can be closed with the laser. Larger ones may require several treatments. They turn dark, dry up, and are usually gone.
There are other uses for lasers but that about covers the more common things and I'm worn out with it. We have covered how lasers work, hair removal, skin rejeuvenation, abnormal coloration, and vascular lesions....we treat them all. Something new next week.
